- Contact the Data Protection Office (DPO)
Reach out to the DPO via email at dpo@mseuf.edu.ph or visit the Legal Office, located on the ground floor of the AEC Building (beside the IGSR Conference Room). - Accomplish the Access Request Form (ARF)
Complete the Access Request Form. Be sure to clearly and accurately describe the purpose of your request in the designated section, including specific details of the information you are seeking.

Submit the duly accomplished form along with a photocopy of a valid ID (e.g., student or employee ID) of the subject involved. You may also be asked to provide other supporting documents that justify your request.
Note: Supporting requirements vary on a case-to-case basis, depending on the purpose of your request. - Await Review and Approval from the DPO
The DPO will review your request and may contact you for further clarification or a short interview regarding your purpose. Once approved, your request will be forwarded to the appropriate department that holds the requested information. - Release of Information
The concerned department will release the requested information either via email or as a hard copy. The DPO will be notified once the information has been provided to the requesting party.
