Inquiry Summary Form
This service allows individuals to submit inquiries, requests for clarification, or concerns related to data privacy and personal data processing. All inquiries are documented and evaluated by the Data Protection Office DPO to ensure timely and appropriate responses in accordance with the Data Privacy Act of 2012 RA 10173.
To facilitate the proper handling of inquiries and maintain accountability, please follow the steps below when submitting a data privacy-related inquiry:
- Complete the Inquiry Summary Form
Fill out the necessary details in the Inquiry Summary Form, including your personal information, the specifics of the inquiry (e.g., the type of personal data involved and a description of the inquiry), and sign the Consent and Declaration section.

You may submit the accomplished Microsoft Form online or physically submit a printed copy of the form to the Data Protection Office, located at the Legal Counsel Office, Ground Floor, AEC Building (beside the IGSR Conference Room). - Evaluation and Preparation of Response
Upon receipt of the inquiry, the Data Protection Office (DPO) shall evaluate the nature of the inquiry or complaint and coordinate with the concerned offices, if necessary, to prepare an appropriate response. - Notification to the Data Subject
The Data Protection Office shall notify the data subject of the status or outcome of the inquiry within three (3) working days from the receipt of the accomplished form, subject to the complexity and nature of the concern.
