"A Web-based Medical Information and Disease Monitoring System, is intended for barangay health center in Pagbilao. Quezon. The purpose is to modernize the existing manual records of the patients in this area. It helps barangay health workers in reducing and printing of papers, photocopying and updating all those records in the modern technical way. Web-based Medical Information and Disease Monitoring System that can help retrieved information of patients and view a summary of disease. It maintains administrator and registered staff that will have a complete access of the functionalities of the system. The administrator have the capability to create or add a staff account, add and update the patient records and also the admin can view or manage the reports. The staff can add and update the patient records. Descriptive Analytics is used in the developed system to better understand the attribute and reports in analytics. With the use of graphs, municipal health officer can easily identify the different disease or the leading diseases of the residence in Pagbilao, Quezon. Also can easily compare and analyze diseases rampant in their existing locality"
