PRISM Equipment Borrowing Form

This form is intended for PRISM Student Envergan Ambassadors who wish to borrow digital content equipment from the Center for Marketing Innovation and Digital Engagement CMIDE for authorized university-related content production, documentation, campaigns, and activities.Borrowers are expected to handle all equipment responsibly, use them only for their approved purpose, and return them complete, clean, and in good working condition within the agreed borrowing period.
