Application to Hold In – and – Off Campus Activities

  • Student organizations need to file an application in triplicate to hold in-campus activities that include the following requirements: (1) letter of adviser/s: (2) list of participants including officers; (3) agenda (for meetings); (4) program, mechanics and committees (for programs and seminars); (5) official permit for the use of campus facility/ies including the letter requesting permission to use the facility/ies in a department and the approval of the concerned department.
  • Student organizations need to file an application in triplicate to hold off-campus activities that include the following requirements: (1) letter of adviser/s: (2) list of participants including officers; and (3) waivers of all participants including the officers.
  • The application needs to be endorsed by the OSO Coordinator to the OSA Director.
  • Once an in- or off-campus activity is approved, the officers and members need to submit narrative and financial reports five days after the conduct of the approved activity to the Coordinator, Office of Student Organizations.
